Family doctors warn: mandatory psychiatric e-consultation will overburden primary care
The Estonian Association of Family Doctors warns that the planned mandatory e-consultation system in psychiatry will not improve patient access to mental health services but will instead overload primary care providers. Social Affairs Minister Karmen Joller defends the reform, saying its goal is to direct the most severe cases to psychiatrists. The two parties disagree on whether the system is ready for the change.
EstoniaThe Estonian Association of Family Doctors (EPS) issued a sharp public statement in early June warning of the risks posed by the planned psychiatric e-consultation system. In the association's view, the mandatory e-consultation requirement will not make mental health services more accessible but will place excessive burden on family doctors' shoulders.
Social Affairs Minister Karmen Joller disagrees with this assessment. According to the minister, the main objective of the reform is to ensure that psychiatrists' limited appointment slots reach first and foremost those in the most difficult circumstances and who cannot wait for better conditions. Joller emphasizes that the association cannot afford to wait for a perfect support system to be in place when people need help now.
The EPS, however, argues that family doctors' already strained schedules cannot absorb new mandatory duties without additional resources and clear organizational procedures. The association warns that implementing mandatory e-consultation without strengthening primary care could result in a situation where neither psychiatrists nor family doctors can adequately serve patients.
The dispute reflects broader tension within Estonia's healthcare system: on one hand, growing mental health problems are putting pressure on the system to change; on the other hand, primary care lacks the means to safely accommodate such changes. Both sides acknowledge that access to mental health services needs improvement, but they disagree on the order and manner in which to implement the changes.
